Overview

Acetyl-DL-leucine + memantine is a combination of two small molecule drugs used experimentally for neurological disorders. Acetyl-DL-leucine (also known as Tanganil) is a modified amino acid that has shown potential neuroprotective and symptomatic benefits in various neurological conditions, including ataxia and lysosomal storage disorders. Memantine is an N-methyl-D-aspartate (NMDA) receptor antagonist approved for the treatment of moderate to severe Alzheimer's disease; it works by blocking excessive glutamatergic activity, which can be neurotoxic. The combination has been reported to improve symptoms such as acquired pendular nystagmus, oscillopsia, stance, and gait in patients with multiple sclerosis in case reports. The mechanism likely involves both modulation of neuronal excitability via NMDA receptor antagonism (memantine) and possible stabilization of neuronal function or reduction of neuroinflammation (acetyl-DL-leucine)[1][2][3][4].
