Acetyl-L-carnitine + nicotinamide is a combination of two small molecules—acetyl-L-carnitine, an acetylated form of the amino acid derivative L-carnitine, and nicotinamide (vitamin B3). Acetyl-L-carnitine facilitates the transport of fatty acids into mitochondria for β-oxidation and energy production. Nicotinamide is a precursor to NAD+, a critical coenzyme in cellular metabolism and mitochondrial function. The combination has been studied primarily in preclinical models for its potential to improve metabolic parameters, support mitochondrial health, modulate immune responses, and promote pancreatic beta-cell function. In murine models of type 1 diabetes induced by streptozotocin, this combination led to remission of hyperglycemia and restoration of insulin secretion[1]. Mechanistically, it acts by enhancing mitochondrial energy metabolism (via increased fatty acid oxidation and NAD+ synthesis), reducing oxidative stress, supporting insulin secretion from pancreatic beta cells, and modulating immune-mediated damage.
