Acetyl-L-carnitine + simvastatin is a combination of two agents used primarily for their complementary effects on lipid metabolism and cardiovascular risk. Simvastatin is an HMG-CoA reductase inhibitor (statin) that lowers cholesterol by inhibiting the conversion of HMG-CoA to mevalonate, a key step in cholesterol biosynthesis. This results in reduced plasma levels of LDL-cholesterol and triglycerides, and increased HDL-cholesterol[6]. Acetyl-L-carnitine (a derivative of L-carnitine) is involved in mitochondrial fatty acid transport and oxidation, supporting energy production and exhibiting antioxidant properties[2][4]. The combination has been studied for enhanced antihyperlipidemic effects, including improved lipid profiles and reduction in lipoprotein(a) [Lp(a)] levels compared to statin monotherapy[1][3]. Additionally, L-carnitine may mitigate statin-induced myopathy by protecting against mitochondrial dysfunction and oxidative stress[5][8].
