Overview

N-acetylcysteine + silibin is a combination of two antioxidant nutritional supplements being investigated for the treatment of diabetic nephropathy in patients with Type 2 Diabetes Mellitus (T2DM). N-acetylcysteine (NAC) serves as a precursor to glutathione, a critical endogenous antioxidant, while silibin (silibinin) is a flavonolignan derived from milk thistle (Silybum marianum) that exhibits potent antioxidant and anti-inflammatory properties. In clinical studies, silibin is often formulated as a phosphatidylcholine complex (Siliphos) to improve its oral bioavailability. The combination is designed to target oxidative stress and inflammation pathways, aiming to reduce proteinuria and systemic markers of kidney damage. This therapeutic regimen is being evaluated in Phase 2 clinical trials sponsored by the VA Office of Research and Development.
