Overview

Acetyldigoxin is a cardiac glycoside and an acetyl derivative of digoxin, primarily used to treat acute and chronic heart failure as well as certain arrhythmias such as atrial fibrillation. It exerts its effects by increasing the force of myocardial contraction (positive inotropy) while reducing heart rate and conduction velocity through the atrioventricular node. The mechanism of action involves inhibition of the sodium/potassium-transporting ATPase subunit alpha-1 (Na+/K+ ATPase) in cardiac myocytes, leading to increased intracellular sodium and subsequent elevation of intracellular calcium via reduced activity of the sodium-calcium exchanger. This results in enhanced contractility without a significant increase in energy expenditure by the heart. Acetyldigoxin is better absorbed and has a longer duration of action than digoxin[1][2][3][5][7].
