acidic oligopeptide-DOTA-177Lu (also known as Lu177-LT17) is an investigational, bone-targeted radioligand therapy being developed by the Low Institute for Therapeutics (LIFT) in collaboration with NorthStar Medical Radioisotopes. The therapeutic agent consists of a bone-homing acidic oligopeptide ligand conjugated via a stable linker to a DOTA chelator, which complexes the beta-emitting radionuclide lutetium-177 (177Lu). The acidic oligopeptide selectively targets exposed hydroxyapatite, a mineral component of bone that becomes exposed in both osteolytic and osteoblastic bone lesions but remains occluded by a cartilaginous sheath in healthy bone. Upon binding to hydroxyapatite at the sites of bone metastases, the conjugate delivers localized, cytotoxic beta radiation directly to the tumor microenvironment, inducing DNA damage and tumor cell death while minimizing off-target toxicity to healthy tissues and bone marrow. Lu177-LT17 is currently undergoing IND-enabling studies for the treatment of cancer-associated bone disease, including bone metastases arising from breast, prostate, and lung cancers, as well as hematologic malignancies.
