Acipimox is a small molecule niacin (nicotinic acid) derivative used as a hypolipidemic agent. It acts primarily by inhibiting the release of free fatty acids from adipose tissue, which reduces hepatic production of triglycerides and very low-density lipoprotein (VLDL). This leads to decreased serum levels of VLDL and low-density lipoprotein (LDL), with a modest increase in high-density lipoprotein (HDL). Acipimox is mainly indicated for the treatment of various types of hyperlipoproteinemia, including Fredrickson type IIb and IV. It has also demonstrated beneficial effects on glucose metabolism in patients with adult-onset diabetes and may reduce the risk of coronary heart disease. Adverse effects are generally milder than those seen with standard doses of nicotinic acid but can include flushing, palpitations, gastrointestinal disturbances, impaired glucose tolerance at high doses, liver function disorders, and rarely gout[2][5][10].
