Aclimostat is a small molecule drug and a second-generation methionine aminopeptidase 2 (MetAP2) inhibitor developed for the treatment of type 2 diabetes mellitus and obesity. It belongs to the fumagillin-class of compounds and is administered as a twice-weekly injectable. The drug acts by inhibiting MetAP2, an enzyme involved in cotranslational removal of N-terminal methionine from nascent proteins, which plays a role in metabolic regulation. Inhibition of MetAP2 has been shown to improve glucose control and promote weight loss in overweight or obese individuals with type 2 diabetes. Aclimostat was originally developed by Zafgen and later by Larimar Therapeutics. Clinical trials have reached Phase II for both type 2 diabetes mellitus and obesity[1][3][4][6].
