acoltremon

Acoltremon (brand name Tryptyr, also known as AR-15512) is a first-in-class small molecule agonist of the transient receptor potential melastatin 8 (TRPM8) receptor, developed for the treatment of dry eye disease. Originally developed by Aerie Pharmaceuticals and subsequently acquired by Alcon, the drug is formulated as a 0.003% ophthalmic solution for twice-daily administration. It functions as a neuromodulator that stimulates cold-sensing TRPM8 receptors on corneal sensory nerves, thereby activating trigeminal nerve signaling to increase natural basal tear production. Acoltremon received FDA approval in May 2025 based on the results of the COMET-2 and COMET-3 Phase 3 clinical trials, which demonstrated a significant and rapid increase in tear production.
