Overview

Acotiamide is a first-in-class oral prokinetic drug used primarily for the treatment of functional dyspepsia, specifically postprandial distress syndrome (PDS) symptoms such as bloating after meals, epigastric bloating, and early satiety. It was first approved in Japan in 2013 and is also available in Russia. Acotiamide acts by enhancing upper gastrointestinal motility through two main mechanisms: antagonism of muscarinic M1 and M2 autoreceptors in the enteric nervous system (which increases acetylcholine release), and inhibition of acetylcholinesterase activity. These actions improve gastric accommodation to meals and accelerate gastric emptying. Acotiamide may also have effects on brain-gut interactions via modulation of vagal pathways[2][3][6][8].
