ACQT1127 is an orally bioavailable small molecule being developed by Acquist Therapeutics for the treatment of gout and nonalcoholic steatohepatitis (NASH). The drug utilizes a dual-action mechanism that simultaneously targets enzymes regulating both the production and excretion of uric acid, demonstrating significantly higher potency compared to standard monofunctional agents. In preclinical studies for NASH, ACQT1127 has shown the ability to renormalize dysregulated levels of seven key activators, including AMP-activated protein kinase alpha (AMPKα), Acetyl-CoA carboxylase (ACC), Sterol regulatory element-binding protein 1 (SREBP1), and ATP-citrate synthase (ACLY). Preclinical data in human liver cells indicate that ACQT1127 can block uric acid increases by over 98% and reduce triglyceride levels by more than 50%, addressing both the metabolic and inflammatory components of liver disease and hyperuricemia.
