Overview
Acthar Gel + tacrolimus is a combination therapy investigated for the treatment of steroid-resistant nephrotic syndrome (SRNS), including membranous glomerulopathy (MGN) and focal segmental glomerulosclerosis (FSGS). Acthar Gel (repository corticotropin injection) is a porcine-derived biologic containing adrenocorticotropic hormone (ACTH) and other proopiomelanocortin (POMC) peptides. It functions as an agonist for melanocortin receptors (MCRs), notably MC2R in the adrenal cortex to stimulate cortisol release and MC1R on glomerular podocytes to reduce proteinuria. Tacrolimus is a small molecule calcineurin inhibitor that suppresses T-cell activation and stabilizes the podocyte cytoskeleton. This combination approach, studied by the Southeast Renal Research Institute, aims to provide synergistic efficacy in patients who have failed standard glucocorticoid therapy, potentially reducing the need for high-dose steroids and their associated morbidities.
