Actimab-A + CLAG-M is a combination therapeutic regimen developed by Actinium Pharmaceuticals for the treatment of relapsed or refractory acute myeloid leukemia (AML). Actimab-A is an antibody radio-conjugate (ARC) consisting of the anti-CD33 monoclonal antibody lintuzumab conjugated to the alpha-emitting radioisotope actinium-225 (Ac-225). CLAG-M is a salvage chemotherapy regimen comprising cladribine, cytarabine (Ara-C), filgrastim (G-CSF), and mitoxantrone. The scientific rationale for the combination is that CLAG-M provides rapid debulking of leukemia cells, while Actimab-A delivers precision CD33-targeted alpha-particle radiation to eliminate residual disease via double-strand DNA breaks. This regimen is particularly targeted at high-risk patients, including those with TP53 mutations, prior venetoclax therapy, or prior allogeneic stem cell transplant, and has demonstrated high rates of MRD-negativity in clinical trials.
