Actinium (225Ac) Ibu-DAB-PSMA (also known as [225Ac]Ac-SibuDAB) is an investigational targeted alpha therapy (TAT) developed by ITM Oncologics for the treatment of PSMA-positive metastatic castration-resistant prostate cancer (mCRPC). The compound consists of the alpha-emitting radioisotope Actinium-225 conjugated to a small-molecule PSMA ligand that has been modified with an ibuprofen-based albumin-binding moiety ('Ibu') and a diaminobutyric acid (DAB) linker. This design is intended to extend the drug's circulation time in the blood, thereby enhancing tumor uptake and therapeutic efficacy compared to first-generation radioligands. Although it entered Phase I clinical development, the trial was withdrawn before completion.
