Overview

Activated donor lymphocyte infusion (ADLI) is an investigational cellular immunotherapy developed at the Abramson Cancer Center at Penn Medicine. The therapy involves the ex vivo expansion and activation of donor-derived T lymphocytes using anti-CD3 and anti-CD28 monoclonal antibody-coated beads. This costimulation process is designed to enhance the T cells' proliferative capacity and anti-tumor effector functions. ADLI is administered to patients with high-risk hematologic malignancies, including acute myeloid leukemia (AML), acute lymphoblastic leukemia (ALL), and myelodysplastic syndromes (MDS), typically in a prophylactic setting following non-myeloablative allogeneic stem cell transplantation. The primary therapeutic goal is to augment the graft-versus-leukemia (GVL) effect to eliminate residual disease and prevent relapse while maintaining a manageable safety profile regarding graft-versus-host disease (GVHD).
