Activated PTCy-MILs is an autologous adoptive T-cell therapy (specifically donor-derived cells harvested from the recipient) developed by the Sidney Kimmel Comprehensive Cancer Center at Johns Hopkins. The therapy consists of marrow-infiltrating lymphocytes (MILs) harvested from the bone marrow of patients who have relapsed with hematologic malignancies after undergoing allogeneic hematopoietic cell transplantation (alloHCT) using post-transplantation cyclophosphamide (PTCy) as graft-versus-host disease (GVHD) prophylaxis. These MILs are activated and expanded ex vivo using anti-CD3 and anti-CD28 monoclonal antibodies covalently attached to super-paramagnetic microbeads. This process is designed to enhance the graft-versus-tumor (GVT) effect by leveraging tumor-reactive T cells that have survived the PTCy treatment environment, potentially offering a therapeutic option for post-transplant relapse with a reduced risk of severe GVHD.
