Overview

Active fluid management with BIA monitoring is a clinical therapeutic protocol developed by Seoul St. Mary's Hospital to optimize the hydration status of postoperative patients in the Surgical Intensive Care Unit (SICU). The strategy utilizes real-time Bioelectrical Impedance Analysis (BIA) to monitor the extracellular water (ECW) ratio, targeting a specific range of 0.390 to 0.406. Patients identified as dehydrated (ECW ratio < 0.390) receive intravenous bolus infusions of PlasmaLyte, a balanced crystalloid solution used for volume expansion. Patients identified as overhydrated (ECW ratio > 0.406) are administered furosemide, a loop diuretic that inhibits the NKCC2 symporter in the thick ascending limb of the loop of Henle. This guided approach aims to maintain patients within an ideal fluid status range to improve clinical outcomes and prevent complications associated with fluid imbalance in the acute postoperative phase.
