ACVBP is an intensive combination chemotherapy regimen used primarily for the treatment of aggressive non-Hodgkin lymphoma, particularly diffuse large B-cell lymphoma (DLBCL). The acronym stands for its component drugs: doxorubicin (Adriamycin), cyclophosphamide, vindesine, bleomycin, and prednisone. In some protocols, rituximab is added to form the R-ACVBP or ACVBP-R regimen. The mechanism of action involves multiple cytotoxic agents with different targets and mechanisms—doxorubicin intercalates DNA and inhibits topoisomerase II; cyclophosphamide alkylates DNA; vindesine disrupts microtubule formation; bleomycin induces DNA strand breaks; prednisone is a corticosteroid with lympholytic effects. This multi-agent approach aims to maximize tumor cell kill and reduce resistance. ACVBP has been shown in clinical trials to improve event-free survival and overall survival compared to standard CHOP therapy in certain high-risk patient populations but at the cost of increased toxicity[1][3][4].
