**AD-114** is a fully humanized single-domain antibody-like scaffold (i-body) derived from shark IgNAR antibodies, developed by AdAlta as a selective antagonist of the G protein-coupled receptor **CXCR4**. It penetrates deeply into the CXCR4 ligand-binding pocket to block signaling pathways including p38 MAPK and PI3K/AKT/mTOR, inhibiting SDF-1/CXCR4-mediated chemotaxis of inflammatory cells and fibrocytes without mobilizing stem cells, unlike small-molecule antagonists like AMD3100. Preclinical studies demonstrate potent anti-fibrotic and anti-inflammatory effects, reducing extracellular matrix deposition, TGF-β1 expression, macrophage infiltration, and fibroblast activation in mouse models of renal fibrosis (folic acid nephropathy and unilateral ureteral obstruction) and pulmonary fibrosis (bleomycin-induced), positioning it as a promising first-in-class biologic for fibrotic diseases.[1][2][5][8]
