Overview

AD-118 is an incrementally modified drug (IMD) formulation of the combination sacubitril and valsartan, developed by Addpharma Inc. for the treatment of chronic heart failure. It belongs to the Angiotensin Receptor-Neprilysin Inhibitor (ARNI) class. Sacubitril is a prodrug that, upon activation to sacubitrilat, inhibits the enzyme neprilysin, thereby increasing the levels of beneficial natriuretic peptides. Valsartan is an angiotensin II receptor blocker (ARB) that inhibits the renin-angiotensin-aldosterone system by antagonizing the AT1 receptor. This dual mechanism of action promotes vasodilation, natriuresis, and diuresis while inhibiting maladaptive cardiovascular remodeling, making it a potent therapy for managing heart failure symptoms and reducing cardiovascular mortality.
