Ad-IL-18 is an investigational gene therapy consisting of a replication-deficient adenoviral vector (typically serotype 5) engineered to express the human interleukin-18 (IL-18) gene. IL-18 is a potent pro-inflammatory cytokine belonging to the IL-1 family that induces the production of interferon-gamma (IFN-γ) and enhances the cytolytic activity of natural killer (NK) cells and cytotoxic T lymphocytes (CTLs). When administered, usually via intratumoral injection, the vector infects tumor cells and local stroma, leading to the local production of IL-18. This creates an immunostimulatory microenvironment intended to overcome tumor-induced immunosuppression and promote a systemic anti-tumor immune response. It has been primarily investigated in early-phase clinical trials for the treatment of advanced solid tumors, including melanoma and various carcinomas.
