Ad26.RSV.preF is a viral vector vaccine developed by Janssen (a subsidiary of Johnson & Johnson) for the prevention of respiratory syncytial virus (RSV) infection. It utilizes a replication-deficient adenovirus type 26 (Ad26) vector to deliver the genetic sequence for the RSV fusion (F) protein, which is stabilized in its prefusion conformation (preF). The prefusion F protein is the primary target for neutralizing antibodies against RSV. The vaccine was designed to induce both robust humoral (antibody) and cellular (T-cell) immune responses. While it showed promising efficacy in Phase 2b trials (CYPRESS), Janssen announced the discontinuation of its adult RSV vaccine program, including the Phase 3 EVERGREEN trial, in March 2023 as part of a strategic portfolio prioritization and an assessment of the competitive landscape.
