Adalimumab + certolizumab pegol is a combination of two monoclonal antibody-based biologic drugs, both of which are tumor necrosis factor-alpha (TNF-alpha) inhibitors. Adalimumab is a fully human monoclonal antibody that binds to TNF-alpha, preventing its interaction with cell surface TNF receptors and thereby reducing inflammation. Certolizumab pegol is a PEGylated, Fc-free anti-TNF-alpha antibody fragment (Fab') that also neutralizes TNF-alpha activity but lacks the Fc region, potentially reducing certain immune-mediated effects. Both agents are approved individually for the treatment of autoimmune inflammatory diseases such as rheumatoid arthritis, psoriatic arthritis, ankylosing spondylitis, Crohn’s disease, and plaque psoriasis. The combination itself does not have regulatory approval as a fixed-dose product or established clinical use; however, switching between these agents or sequential use has been studied in patients with inadequate response to one agent[7][8][9]. There is no published evidence supporting routine simultaneous co-administration of both drugs together.
