Overview

This is a hypothetical combination of four tumor necrosis factor (TNF) inhibitors—adalimumab, certolizumab pegol, etanercept, and infliximab. Each of these agents is a biologic monoclonal antibody or fusion protein that targets and neutralizes TNF-alpha, a pro-inflammatory cytokine involved in the pathogenesis of several autoimmune diseases. These drugs are individually approved for the treatment of conditions such as rheumatoid arthritis, psoriatic arthritis, ankylosing spondylitis, Crohn’s disease, ulcerative colitis, and plaque psoriasis. They work by binding to TNF-alpha and preventing it from activating its receptor on cell surfaces, thereby reducing inflammation and immune-mediated tissue damage[2][3][7]. There is no clinical precedent or regulatory approval for using all four agents together in combination due to overlapping mechanisms of action and increased risk for serious adverse effects such as infections.
