Overview

This is a combination of four biologic disease-modifying antirheumatic drugs (DMARDs): adalimumab, etanercept, golimumab, and tocilizumab. Each is a monoclonal antibody or receptor fusion protein targeting key cytokines involved in inflammatory pathways. Adalimumab, etanercept, and golimumab are tumor necrosis factor-alpha (TNF-α) inhibitors that block the activity of TNF-α, a pro-inflammatory cytokine central to the pathogenesis of rheumatoid arthritis and other autoimmune diseases. Tocilizumab is an interleukin-6 (IL-6) receptor antagonist that inhibits IL-6-mediated signaling involved in inflammation and immune response. These agents are individually approved for use in moderate-to-severe rheumatoid arthritis and other autoimmune conditions; however, their combined use as a four-drug regimen is not standard clinical practice due to increased risk of immunosuppression and adverse effects[1][2][9]. Combination therapy with multiple biologics is generally avoided because of safety concerns.
