The combination of adalimumab and prednisone represents a therapeutic approach where these two medications are used together to treat various inflammatory conditions. This is not a fixed-dose combination product but rather two separate medications often prescribed concurrently. ## Drug Interaction Information Adalimumab is a tumor necrosis factor (TNF) blocker that works by suppressing the immune system to reduce inflammation in conditions like rheumatoid arthritis, psoriasis, Crohn's disease, and ulcerative colitis. Prednisone is a corticosteroid that also reduces inflammation through a different mechanism. When used together, these medications have important interaction considerations: - A clinically significant interaction exists between adalimumab and prednisone. - Concomitant use of corticosteroids (like prednisone) with adalimumab is associated with a significantly increased risk of serious infections (HR 2.40; 95% CI 1.33–4.350, p=0.004). - This increased infection risk has been found across different anti-TNF agents and various inflammatory conditions, including rheumatoid arthritis, psoriasis, and spondyloarthropathies. ## Clinical Outcomes Despite the increased infection risk, there are potential benefits to the combination: - In Crohn's disease studies, higher rates of remission were observed in patients receiving anti-TNF therapy plus corticosteroids compared to most prior studies of anti-TNF therapies alone. - However, patients on steroids plus anti-TNF were less likely to achieve clinical response than those on anti-TNF alone (42.7% vs. 46.8%, respectively). - After controlling for baseline disease activity, there was no significant difference in odds of clinical response between the two groups. ## Steroid Reduction with Adalimumab One important benefit of adalimumab therapy is its potential to reduce or eliminate the need for steroids: - In a study of rheumatoid arthritis patients from the CORRONA registry, patients who initiated adalimumab showed significant reductions in prednisone use after 6 months. - The mean steroid dose decreased from 6.1 mg at baseline to 3.6 mg after 6 months of adalimumab therapy. - Approximately 53% of patients who were on steroids at adalimumab initiation were able to discontinue steroids entirely by 6 months. ## Recommendations Healthcare providers typically consider the risk-benefit profile when prescribing these medications together: - The combination may be used in certain inflammatory conditions where the benefits outweigh the risks. - Patients should be closely monitored for signs of infection when using both medications. - A goal of treatment is often to reduce or eliminate steroid use over time as the adalimumab takes effect. - Doctors may recommend alternative treatment approaches depending on the specific condition and patient factors.
