Overview

ADAMTS13-CAAR T cells are an experimental chimeric autoantibody receptor (CAAR) T cell therapy designed to treat immune-mediated thrombotic thrombocytopenic purpura (iTTP) by selectively eliminating B cells that produce pathogenic autoantibodies against the von Willebrand factor–cleaving protease ADAMTS13. In this approach, patient T cells are engineered ex vivo to express a synthetic receptor whose extracellular domain consists of ADAMTS13 autoantigen fragments, enabling the modified T cells to bind and kill only those B cells bearing anti-ADAMTS13 B cell receptors, thereby depleting the disease-causing clones while sparing the broader B cell repertoire.[5][8][11][15] Preclinical work from academic groups has demonstrated that ADAMTS13-based CAAR constructs can specifically target anti-ADAMTS13 B cells in models of iTTP, and this platform is under early-stage, preclinical development as a precision immunotherapy for this rare autoimmune thrombotic microangiopathy.[5][8][11]
