Adapentpronitrile is a novel adamantane-based small molecule that functions primarily as a dipeptidyl peptidase-IV (DPP-IV) inhibitor. It has demonstrated hypoglycemic effects and the ability to ameliorate pancreatic β cell dysfunction in models of type 2 diabetes mellitus by inhibiting DPP-IV activity. In addition to its antidiabetic properties, adapentpronitrile has shown neuroprotective effects in diabetic rat models, where it significantly reduced neuronal injury and decreased amyloid precursor protein (APP) and amyloid beta (Aβ) expression in the hippocampus and cortex. The compound exerts its neuroprotective action at least partially by inhibiting mitochondrial oxidative stress and modulating apoptotic pathways—downregulating pro-apoptotic proteins such as BAX, cytochrome c, caspase-9, and caspase-3 while upregulating anti-apoptotic Bcl-2. Notably, some of these protective effects appear to be independent of DPP-IV inhibition[1][3][4].
