Overview

ADC4022 is an inhaled formulation of theophylline, a methylxanthine derivative, developed by Pulmagen Therapeutics (formerly Argenta Discovery) for the treatment of Chronic Obstructive Pulmonary Disease (COPD). While oral theophylline has been used for decades as a bronchodilator, its clinical utility is limited by a narrow therapeutic window and significant systemic side effects. ADC4022 is designed for direct delivery to the respiratory tract to achieve high local concentrations while minimizing systemic exposure. The drug acts as a non-selective phosphodiesterase (PDE) inhibitor and an adenosine receptor antagonist, providing bronchodilation. Additionally, at low doses, theophylline has been shown to activate histone deacetylase 2 (HDAC2), an enzyme that is downregulated in the lungs of COPD patients. By activating HDAC2, ADC4022 aims to restore corticosteroid sensitivity and enhance the anti-inflammatory efficacy of co-administered inhaled corticosteroids like budesonide.
