AdCAR-T cells (Adapter CAR-T cells) are a modular chimeric antigen receptor T-cell immunotherapy platform developed by Miltenyi Biotec in collaboration with the University of Tuebingen. The platform utilizes genetically engineered T-cells that express a universal 'adapter' CAR (AdCAR) designed to recognize a biotin tag within a specific linker-label epitope (LLE). Unlike traditional CAR-T therapies that target a single fixed antigen, AdCAR-T cells are activated only in the presence of biotinylated adapter molecules (AMs)—typically monoclonal antibodies or fragments—that bridge the T-cells to specific tumor-associated antigens. This modularity allows for precise control over CAR-T function via an 'on/off' switch mechanism and enables the simultaneous or sequential targeting of multiple antigens to prevent antigen escape and overcome tumor heterogeneity. The therapy is being investigated for the treatment of acute myeloid leukemia (AML), B-lineage lymphomas, and solid tumors such as metastatic breast cancer.
