AdCD40L is an immunostimulatory gene therapy that uses a replication-deficient adenoviral vector to deliver the CD40 ligand (CD40L) gene to tumor cells. This therapy aims to activate the immune system against cancer by converting the tumor microenvironment from immune-suppressive to immune-active. ## Mechanism of Action AdCD40L works by expressing CD40L within the tumor microenvironment, which activates dendritic cells and other antigen-presenting cells. This activation leads to several important immunological effects: 1. Conversion of the tumor microenvironment from immune-suppressive to immune-inflamed 2. Activation of pro-inflammatory cytokines and chemokines 3. Reduction in immunosuppressive factors like TGF-β 4. Increased infiltration of immune cells, particularly CD4+ T cells 5. Shift from Th2-dominated to Th1-dominated immune responses The therapy has been shown to induce significant antitumor effects in various preclinical models and has entered clinical trials for several cancer types, including melanoma, urothelial cancer, and diffuse midline glioma.
