Adductor Canal Block (ACB) + iPACK Block is a combination regional anesthesia intervention used for postoperative pain management, particularly in patients undergoing total knee arthroplasty (TKA). The ACB involves the injection of a local anesthetic, such as bupivacaine, into the adductor canal to provide sensory blockade of the saphenous nerve. The iPACK (Infiltration between the Popliteal Artery and Capsule of the Knee) block involves infiltrating the space between the popliteal artery and the posterior capsule of the knee to target terminal sensory branches of the sciatic and obturator nerves. This combination is designed to provide comprehensive analgesia to both the anterior and posterior aspects of the knee while sparing motor function, thereby facilitating early mobilization and functional recovery. The primary active pharmaceutical ingredient used in these blocks is bupivacaine, a long-acting local anesthetic that inhibits nerve impulse conduction by blocking voltage-gated sodium channels.
