Adebrelimab + apatinib + irinotecan is a combination chemo-immunotherapy regimen being investigated for the treatment of advanced gastric and gastroesophageal junction (GEJ) adenocarcinoma. The regimen consists of **adebrelimab**, a humanized IgG4 monoclonal antibody that acts as a checkpoint inhibitor by targeting programmed death-ligand 1 (PD-L1); **apatinib**, a small-molecule tyrosine kinase inhibitor (TKI) that selectively targets vascular endothelial growth factor receptor 2 (VEGFR-2) to inhibit tumor angiogenesis; and **irinotecan**, a cytotoxic chemotherapy agent that inhibits DNA topoisomerase I, leading to double-strand DNA breaks and cell death. This triplet combination is designed to provide a synergistic anti-tumor effect by combining immune checkpoint blockade, anti-angiogenic therapy, and traditional chemotherapy. Clinical trials, notably those involving Renji Hospital and Shanghai Jiao Tong University, have explored this combination as a second-line treatment option for patients with advanced gastric cancer who have failed prior therapies.
