Overview

Adebrelimab + retlirafusp alfa + recaticimab + chemotherapy is a neoadjuvant combination regimen being investigated for the treatment of resectable non-small cell lung cancer (NSCLC). The regimen evaluates the efficacy of combining immune checkpoint inhibition—using either the PD-L1 inhibitor adebrelimab or the bifunctional PD-L1/TGF-β fusion protein retlirafusp alfa (SHR-1701)—with the PCSK9 inhibitor recaticimab (SHR-1209) and standard platinum-based chemotherapy. The rationale for this combination is based on preclinical evidence suggesting that PCSK9 inhibition can synergize with PD-1/PD-L1 inhibitors to suppress tumor growth, potentially by enhancing MHC-I expression and T-cell infiltration. The addition of TGF-β blockade via retlirafusp alfa aims to further overcome immune evasion in the tumor microenvironment. This multi-drug approach is currently in Phase II clinical development.
