Adiphenine is a small molecule anticholinergic agent originally introduced as a spasmolytic and local anesthetic. It acts as an antagonist of both nicotinic and muscarinic acetylcholine receptors. Its mechanism involves reducing the frequency of acetylcholine-induced single-channel currents and accelerating desensitization from the open state of the receptor. Adiphenine has been used to reduce muscle tone in the gastrointestinal tract, bile duct and gallbladder, bronchi, bladder; it also affects ocular muscles (causing mydriasis), increases intraocular pressure, induces tachycardia by influencing cardiovascular function, and improves AV conduction. Clinically it has been employed for symptomatic relief in gastrointestinal disorders characterized by spasms (such as gastric/duodenal ulcers), cholelithiasis (gallstones), urinary stones/colic/dysuria/neurogenic bladder conditions. Side effects include nausea, vomiting, heartburn, dizziness and headache. Adiphenine is not widely used clinically today[1][4][5][6].
