Overview

Adiponectin-transfected endothelial progenitor cells (adiponectin-transfected EPCs) are a form of gene-modified cell therapy in which endothelial progenitor cells (EPCs) are genetically engineered to overexpress adiponectin, an adipocyte-derived cytokine with insulin-sensitizing and anti-inflammatory properties. This modification is typically achieved using a lentiviral vector encoding the adiponectin gene. The resulting LV-APN-EPCs have been studied primarily in preclinical models for their protective effects against cerebral ischemia-reperfusion injury, particularly in the context of type 2 diabetes mellitus (T2DM). Mechanistically, these modified EPCs enhance angiogenesis and vascular repair by increasing microvessel density, reducing infarct size and apoptosis after stroke, and improving neurological outcomes. The beneficial effects are thought to be mediated through increased activation of pathways such as eNOS signaling and upregulation of pro-angiogenic factors like VEGF[1][2][3][5]. These studies suggest potential applications for ischemic stroke and possibly other vascular or neurodegenerative conditions.
