Adipose-derived mesenchymal stromal cells (ASCs) combined with a fibrin scaffold is an investigational cell therapy developed by the **Instituto de Investigación Sanitaria de la Fundación Jiménez Díaz** for the treatment of chronic wounds, including pressure ulcers and vascular ulcers. The therapy involves the topical application of autologous or allogeneic ASCs, which are multipotent cells harvested from adipose tissue, delivered to the wound bed within a fibrin biological carrier. The mechanism of action is primarily paracrine; the ASCs secrete a wide array of pro-regenerative cytokines and growth factors, such as vascular endothelial growth factor (VEGF), hepatocyte growth factor (HGF), and transforming growth factor-beta (TGF-β). These factors promote angiogenesis, modulate the local inflammatory environment, and stimulate the migration and proliferation of resident fibroblasts and keratinocytes to facilitate wound closure. The fibrin component acts as a supportive matrix that enhances cell retention and provides a scaffold for tissue remodeling.
