Adipose-derived stem cells (ASCs) are autologous mesenchymal stem cells isolated from a patient's fat tissue, typically via liposuction. In the context of ophthalmic therapy, specifically for dry macular degeneration, these cells are processed and administered via intravitreal injection. The therapeutic rationale is based on paracrine signaling, where the transplanted cells secrete cytokines, growth factors, and exosomes that exert neuroprotective, anti-inflammatory, and antioxidant effects to support retinal cell survival. A specific implementation by Bioheart (later U.S. Stem Cell), branded as AdipoCell, was investigated in clinical trials (e.g., NCT02024269). However, the approach faced significant scrutiny following reports in the New England Journal of Medicine documenting severe adverse events, including retinal detachment and permanent vision loss, in patients treated at unregulated stem cell clinics.
