Adipose tissue-derived mesenchymal stromal cells (AD-MSCs) are a population of multipotent, non-hematopoietic cells isolated from adipose tissue, typically via liposuction or lipoaspirate. They are characterized by the expression of surface markers such as CD73, CD90, and CD105, and the ability to differentiate into adipogenic, osteogenic, and chondrogenic lineages. AD-MSCs exert their therapeutic effects primarily through immunomodulatory, anti-inflammatory, and regenerative paracrine activities rather than a single discrete molecular target. They secrete a variety of cytokines, chemokines, and growth factors (the secretome) that modulate the local microenvironment, suppress aberrant immune responses (such as T-cell proliferation), and promote tissue repair. AD-MSCs are being investigated for a wide range of conditions, including complex perianal fistulas in Crohn's disease (where the product darvadstrocel is approved), orthopedic disorders, cardiovascular diseases, and autoimmune conditions.
