Overview

Adizem-SR is a sustained-release oral capsule formulation of diltiazem hydrochloride, a benzothiazepine calcium channel blocker. Developed by Napp Pharmaceuticals, it is indicated for the management of angina pectoris and the treatment of mild to moderate hypertension. The drug acts as a calcium antagonist by inhibiting L-type (voltage-gated) calcium channels, thereby blocking the transmembrane entry of calcium ions into myocardial and vascular smooth muscle cells. This inhibition leads to peripheral and coronary vasodilation, reduced heart rate, and decreased cardiac workload. The sustained-release formulation utilizes prolonged-release microgranules to provide therapeutic effects over a 12-hour period, typically requiring twice-daily dosing. It is designed to be swallowed whole to maintain its release profile.
