Overview

Adrenomedullin pegol is a PEGylated form of the endogenous vasodilatory peptide hormone adrenomedullin, engineered by covalent attachment to a polyethylene glycol polymer to prolong systemic half-life and enable inhaled delivery to the lung. It is being (and was) developed as an inhalation therapy to replace or augment adrenomedullin activity in the pulmonary vasculature, aiming to enhance endothelial barrier function, reduce vascular hyperpermeability, and thereby decrease extravascular lung water and improve oxygenation in acute respiratory distress syndrome (ARDS) and related lung failure conditions.[1][7][9] In a phase 2a/b randomized, double‑blind, placebo‑controlled trial in mechanically ventilated ARDS patients, inhaled PEG‑ADM at 960 μg and 1920 μg once daily for up to 10 days was well tolerated but failed to improve ventilator‑free survival or composite efficacy endpoints compared with placebo, leading Bayer to terminate further development in ARDS for futility despite an acceptable safety profile.[1][2][4][7][11]
