ADSCs-Exo + Sflt-1 is an experimental, preclinical gene-loaded exosome biologic designed to treat rheumatoid arthritis-associated interstitial lung disease (RA-ILD). It consists of exosomes derived from adipose-derived stem cells (ADSCs) engineered to deliver the soluble vascular endothelial growth factor receptor 1 (Sflt-1) gene. Sflt-1 acts as a decoy receptor that binds to and antagonizes vascular endothelial growth factor (VEGF), thereby inhibiting VEGF-mediated angiogenesis and inflammation. In preclinical mouse models, this therapy has demonstrated the ability to alleviate both joint inflammation and pulmonary fibrosis/inflammation by reducing pro-inflammatory cytokines (IL-1β, IL-6, IL-17A) and pro-fibrotic factors (TGF-β, VCAM).
