Adu-8D3 (mAbAdu-scFab8D3) is a bispecific monoclonal antibody engineered as a brain-shuttle theranostic for the treatment and diagnosis of Alzheimer's disease. It is composed of the amyloid-beta (Aβ)-targeting antibody aducanumab fused to a single-chain antibody fragment (scFab) of the 8D3 antibody, which binds to the transferrin receptor 1 (TfR1). This dual-targeting approach utilizes the TfR1-mediated transcytosis pathway to facilitate the transport of the antibody across the blood-brain barrier (BBB), significantly increasing brain uptake compared to conventional monoclonal antibodies. When radiolabeled with Zirconium-89 (89Zr), Adu-8D3 functions as an immuno-PET imaging agent, allowing for the visualization and quantification of Aβ plaque pathology in the brain. Preclinical studies in mouse models have demonstrated its ability to detect low plaque loads at early stages of disease and achieve high specific uptake in the brain.
