Advanced platelet-rich fibrin (aPRF) is a second-generation autologous platelet concentrate produced by centrifuging whole blood without the use of anticoagulants. Unlike first-generation platelet-rich plasma (PRP), aPRF forms a dense fibrin matrix that traps platelets, growth factors, and leukocytes (specifically neutrophils). This matrix acts as a bioactive scaffold that provides a sustained release of regenerative molecules, including PDGF, TGF-β, and VEGF, over an extended period of up to 28 days. The inclusion of neutrophils contributes to monocyte/macrophage differentiation, tissue repair, and angiogenesis. While traditionally utilized in oral and maxillofacial surgery for bone grafting and socket preservation, aPRF is increasingly investigated for dermatological applications such as alopecia areata and androgenetic alopecia, often requiring micronization techniques to facilitate injection through fine-gauge needles.
