ADX-88178 is a highly selective, potent positive allosteric modulator (PAM) of the metabotropic glutamate receptor type 4 (mGlu4). Developed by Addex Therapeutics, the compound is being investigated for its potential to treat Parkinson's disease (PD) and L-DOPA-induced dyskinesia (LID). By modulating mGlu4 receptors, which are primarily located on the presynaptic terminals of the indirect pathway in the basal ganglia, ADX-88178 aims to restore the balance of neurotransmission that is disrupted in PD. Preclinical studies in rodent and non-human primate models, such as the MPTP-lesioned marmoset, have demonstrated that ADX-88178 can enhance the anti-parkinsonian effects of L-DOPA and reduce peak-dose dyskinesia. However, recent research also suggests that mGlu4 PAMs like ADX-88178 may carry a risk of exacerbating dopaminergic psychosis, necessitating further evaluation of their neuropsychiatric safety profile.
