Overview

AEM-28 is a synthetic 28-amino acid peptide that acts as a mimetic of apolipoprotein E (ApoE). It is designed to combine the amphipathic lipid-binding domain and the receptor-binding domain of ApoE, enabling it to anchor into lipoprotein surfaces and facilitate their clearance from plasma via heparan sulfate proteoglycan (HSPG) receptors in the liver. This mechanism allows for enhanced removal of atherogenic lipoproteins, completing reverse cholesterol transport and reducing cardiovascular risk. Unlike therapies dependent on LDL receptor function, AEM-28 may benefit patients with homozygous familial hypercholesterolemia or severe refractory hypercholesterolemia who lack functional LDL receptors. The drug also exhibits anti-inflammatory properties and has shown significant lipid-lowering activity in early clinical studies[1][3][5][7][8].
