Afimetoran is a small molecule immunomodulator that acts as a dual antagonist of toll-like receptor 7 (TLR7) and toll-like receptor 8 (TLR8). It is being developed primarily for the treatment of systemic lupus erythematosus (SLE), an autoimmune disease characterized by chronic inflammation and tissue damage. Afimetoran selectively inhibits TLR7 and TLR8 signaling, thereby suppressing downstream inflammatory pathways such as NF-kB and IRF, which are implicated in SLE pathogenesis. Preclinical studies have shown that afimetoran can reduce kidney injury markers, autoantibody titers, and improve steroid response in mouse models of lupus. Clinical trials have demonstrated that afimetoran is safe and well-tolerated in patients with cutaneous lupus erythematosus (CLE) and SLE[1][2][3][4][5][6][7][8]. The drug is currently undergoing phase II clinical evaluation.
