Aflibercept + capecitabine is an investigational combination therapy consisting of aflibercept, a recombinant fusion protein that acts as a decoy receptor for vascular endothelial growth factor (VEGF), and capecitabine, an oral prodrug of 5-fluorouracil (5-FU) that inhibits DNA synthesis. Aflibercept blocks angiogenesis by binding VEGF-A, VEGF-B, and placental growth factor (PlGF), thereby preventing these ligands from activating their native receptors on endothelial cells. Capecitabine is converted to 5-FU in the body and interferes with DNA replication in rapidly dividing tumor cells. The combination has been studied primarily in patients with chemorefractory gastrointestinal cancers (including colorectal and gastric cancer) and breast cancer, particularly those unsuitable for more intensive chemotherapy regimens. Clinical trials have demonstrated manageable safety profiles and some anti-tumor activity[2][3][4][5].
