Aflibercept + FOLFIRI is a combination regimen used primarily as a second-line treatment for metastatic colorectal cancer (mCRC) in patients who have progressed after an oxaliplatin-based chemotherapy. Aflibercept is a recombinant fusion protein that acts as a decoy receptor for vascular endothelial growth factor A (VEGF-A), vascular endothelial growth factor B (VEGF-B), and placental growth factor (PlGF), thereby inhibiting angiogenesis by preventing these ligands from binding to their endogenous receptors on endothelial cells. This reduces tumor blood vessel formation and nutrient supply[3][4][5][6]. FOLFIRI is a chemotherapy regimen consisting of folinic acid (leucovorin), fluorouracil (5-FU), and irinotecan; it works by interfering with DNA synthesis in rapidly dividing cancer cells[1]. The addition of aflibercept to FOLFIRI has been shown to improve overall survival in mCRC compared to FOLFIRI alone[3][5].
