Overview

Aflibercept + THR-149 is a combination of two investigational drugs being studied for the treatment of diabetic macular edema (DME), particularly in patients who respond suboptimally to anti-VEGF therapy. Aflibercept is a recombinant fusion protein that acts as a decoy receptor for vascular endothelial growth factor (VEGF), inhibiting its activity and thereby reducing abnormal blood vessel growth and vascular permeability in the retina[1]. THR-149 is a novel, potent plasma kallikrein inhibitor based on a bicyclic peptide scaffold, designed to block the release of bradykinin and reduce retinal edema by targeting the plasma kallikrein–kinin system, which operates independently from VEGF pathways[2][4][5][6]. The combination aims to address both VEGF-dependent and VEGF-independent mechanisms involved in DME pathogenesis. Both agents are administered via intravitreal injection. Clinical trials are ongoing to compare their efficacy individually and potentially in sequence or combination for DME management[3][5][6].
